All lessons must be cancelled at least 24 hours in advance or the lesson

must be paid for. This includes cancellations due to illness.  Advance cancellations cannot be applied to future lessons and will be made up according to the teacher’s availability.

 

Payment is expected at the time of the lesson. Accepted methods of payment are cash or check only . A $5 fee will be charged for late payments and a $25 fee for returned checks.

 

It is the student’s responsibility to arrive on time for their lesson.  Any missed time due to lateness will not be made up,  the lesson will end at the scheduled time accordingly.

 

The teacher reserves the right to discontinue lessons with the student for any reason at any time.

 

 The student must bring a signed copy of this form to his/her first lesson in order to proceed.
